Sasha Bruce Youthwork (SBY) is hiring a bilingual Case Manager for the Philip Reid program in Washington, DC. The role supports youth with care coordination, social services navigation, and individual service planning, Monday–Friday 2 PM–10 PM EST.

Under the Program Manager, you will carry out client assessments, counseling, and detailed documentation while ensuring program rules and safety protocols. A valid driver's license is required.

Qualifications

  • Bachelor's in social work, psychology, or related field OR 3 years' experience with at-risk youth.
  • Bilingual Spanish/English; Latinx experience a plus.
  • Familiar with social services and DC-area support agencies.
  • Strong communication and organizational skills.
  • Full-time exempt; valid driver's license and reliable transportation; background checks required.

Responsibilities

  • Complete funder-specific admission processes and ensure SPDAT assessments are up to date.
  • Create and monitor individual service plans with other Philip Reid staff.
  • Connect clients with social services and relevant resources per plans.
  • Conduct individual counseling sessions.
  • Plan and support life skills workshops and trainings.
  • Provide crisis intervention and on-site counseling when needed.
  • Perform regular house checks and enforce safety rules.
  • Record service documentation within 24–48 hours; maintain confidential files.
  • Attend team meetings, trainings, and community outreach; ensure trainings are completed.
  • Evening/ weekend availability as needed.
  • Other duties as assigned; commitment to ending youth homelessness and racial equity.

George's County, MD, reach over 6,500 runaway, homeless, abused, and neglected youth, and

About Philip Reid

Philip Reid is a joint 90-day shelter and extended transitional housing program for up to 24 youth

through the age of 24. The short-term shelter allows young people to stay up to 90 days before

they are placed with a longer-term housing program through the CAHP system. For the ETH

side, chronically homeless youth can stay up to six years, depending on their age and are

supported with intensive case management services in order establish a level of stability and self-sufficiency.

Position Summary

Under the Program Manager, the bilingual case manager will provide care coordination, social

service navigation and support to clients in Philip Reid. This position is Monday - Friday 2 PM to 10 PM EST.

Qualifications
  • Bachelor's Degree in a Social Work, psychology, or a human services related field OR Three years of experience working with at risk youth, vulnerable populations - preferably homeless populations;
  • MUST be bilingual (Spanish/English); history working with Latinx populations a plus;
  • Familiarity with social services and support agencies in the District;
  • Strong communication and organizational skills required; and,
  • Reporting to the Program Manager, this is a full-time, exempt level position. All candidates must possess a valid driver's license and have reliable transportation; pass the required clearances; and provide any additional information regarding previous records,
Duties and Responsibilities
  • Completes all necessary funder specific admission processes with clients when referred to Philip Reid including intake, program rules and policies (confidentiality, bullying, release of information, etc.); ensures SPDAT assessments are up to date;
  • Using a competency based framework and in collaboration with other staff at Philip Reid - creates, implements and monitors an individual service plan to assist clients in meeting their goals towards self sufficiency;
  • Connects clients with appropriate social services and actively supports them in the navigation of services including vital documentation, food supports, relevant benefits, etc.; connects clients with relevant resources to their individual service plans and actively supports them in cultivating those relationships;
  • Conducts individual counseling sessions with clients on caseload;
  • Supports the reporting, planning and implementation of activity schedule at the program including various life skills, workshops and trainings on relevant topics to clients;
  • Support or provide crisis intervention and on-site counseling when appropriate;
  • Perform regular house check inspections inside the building and in the area immediately outside; monitor facilities and enforce program rules for client safety;
  • Help provides office coverage, answer and record all phone calls and visitors;
  • Complete service documentation in client case records within 24-48 hours of the service; maintain accurate, complete, and confidential case files, written documentation, reports to track client progress and outcomes
  • Participates in regular team consultation meetings, staff meetings, trainings, conferences, regular supervision and community outreach events as required; taking special care to ensure that required trainings are completed annually;
  • Availability in evenings and weekends as needed;
  • Other duties as assigned by the Program Manager, the Chief of Programs or their designee; and,
  • Must have a commitment to ending youth homelessness, racial equity, inclusion, and issues central to Sasha Bruce Youthwork's mission.
