Job summary

Community Connections is seeking a Bilingual Permanent Supportive Housing (PSHP) Case Manager in Washington, DC to handle up to 25 residents across scattered sites. This on-site role requires driving in the community to meet clients and collaborate with DHS referrals.

The position emphasizes bilingual Spanish/English capabilities, strong communication, and a commitment to housing stability. The role includes comprehensive benefits and growth opportunities within CC.

Qualifications

• Bachelor’s degree required in social work, psychology, sociology, counseling, or related field.
• Valid driver’s license and auto insurance required.
• Experience with vulnerable populations preferred.

Responsibilities

• Develop service plans with clients and families, set goals, and implement strategies.
• Maintain individualized housing stability plans shared with providers.
• Provide direct community-based housing services and case management.
• Assist with housing applications and lease processes.
• Conduct monthly home visits and community contacts.
• Document within 48 hours of client contact.
