Job summary

Community Counseling Solutions in Eastern Oregon is seeking a Wraparound Care Coordinator (Bilingual Preferred) to provide coordinated behavioral health services to youth and families in Umatilla County. This 1.0 FTE, non-exempt position reports to the Clinical Supervisor and requires a passion for collaborative, culturally competent care.

The role includes coordinating services with partner agencies, preparing referrals, leading CRT meetings, and ensuring continuity of care across levels.

Qualifications

• Bachelor's degree in a human services field or equivalent experience.
• Ability to obtain certification as a precommitment investigator if varianced.
• Strong communication, organization, and bilingual abilities preferred.

Responsibilities

• Coordinate mental health services with outside agencies and educational services.
• Facilitate Community Resource Team (CRT) meetings and develop service coordination plans.
• Provide case management to children, adults and families; ensure continuity of care across levels of care.
• Assist with coordinating payments for mental health services and track ISA cases every 90 days.
• Perform duties including data submission to regional programs and maintaining client records.
