Access Behavioral Health Services, Inc. in Nampa is seeking professionals with Targeted Care Coordination experience. The role involves building collaborative teams for youth with SED/SPMI within a client-driven, family-centered Wraparound model.

Availability is for a full-time, hourly position (30+ billable hours/week) with extensive initial training and supportive supervision. The team will serve clients across Idaho’s towns, emphasizing privacy and culturally competent care.

Qualifications

• Must be 18 years or older.
• Pass a background check.
• Must have a car and a valid driver’s license.
• Ability to maintain privacy of Protected Health Information (PHI).
• Willingness to serve clients geographically throughout Idaho.
• Adaptable, client-focused in fast-paced conditions with a strong work ethic.
• Be culturally competent, accepting, and supportive with families and partners.
• Capacity to become CANS certified and utilize CANS or other assessment tools.

Responsibilities

• Build teams for youth diagnosed with serious emotional disturbance (SED) or serious persistent mental illness (SPMI).
• Collaborate with families, supports, and providers in a client-driven, family-centered model.
• Follow Wraparound Program requirements for services.
• Work with children and families located in DHW Region 3.
• Engage in care coordination enactment with multidisciplinary teams.
