Job summary

RiseBoro Community Partnership seeks a Tenant Services Coordinator to provide direct care services and referrals to multiple households at the 315 West 94th Affordable Housing Services Program. The role partners with the Assistant Site Director to promote stable living for clients on the caseload.

The position focuses on on-site service coordination, needs assessments, case management, and community referrals, with emphasis on housing subsidies, benefits, and aging in place through partnerships.

Qualifications

• Bachelor’s Degree in social work, psychology, or related field.
• Bilingual in Spanish and English required.

Responsibilities

• Coordinate day-to-day on-site services for ~20-25 households.
• Complete needs assessments and develop tenant-centered support plans.
• Provide case management, advocacy, and referrals to community services.
• Help tenants maintain housing subsidies and benefits.
• Assist tenants in obtaining government benefits and developing informal support networks.
• Educate residents on on-site services and community resources.
• Maintain resident records and monitor services with management staff.
• Collaborate with partners to support aging in place and discharge planning.
