Job summary

Tulsa Health Department seeks dedicated individuals to provide case management for pregnant women and families in Tulsa County under Healthy Start guidelines. You will screen, assess, educate, and determine intervention strategies to improve birth outcomes and reduce infant mortality.

Responsibilities include intake, referrals, coordination with medical/behavioral health, home visits, and ongoing client support. Strong communication and bilingual abilities are valued; travel may be required.

Qualifications

• Bachelor’s degree in social work or related field required; master’s degree preferred.
• Three years of experience as a case manager or related experience with Maternal-Child Health preferred.
• Excellent interpersonal and good communication skills required.
• Ability to obtain cooperation from clients from diverse backgrounds.
• Bilingual – Spanish may be required.
• Licensure as a social worker by the Oklahoma state may be possible.

Responsibilities

• Provide case management services including screening, assessment, and education.
• Determine intervention strategies within Healthy Start protocols for Tulsa County zip codes listed.
• Coordinate with medical, behavioral health, and social services to improve quality of care.
• Complete intake, screening tools, and risk assessments; document thoroughly.
• Make referrals and follow up on missed appointments; conduct home visits.
• Attend Healthy Start trainings and staff meetings; participate in QA activities.
