Job summary

Telecare is seeking a Case Manager II in Oakland, CA to provide direct case management for individuals with mental illness, helping them live more independently in their chosen communities. The role is full-time, on-site, with daytime hours (9:00 AM to 5:30 PM, Monday through Friday).

Qualifications

• Two years of direct services in mental health or community services.
• Associates degree with one year of experience or Bachelor's degree in Social Services.
• Ability to work under direct supervision in a clinical setting.

Responsibilities

• Serve as primary support and case manager for individuals with mental illness.
• Assist clients to live as independently as possible in their chosen community.
• Shifts: Full-Time DAYS, 9:00 AM - 5:30 PM, Mon-Fri.
