JASA in Brooklyn seeks a Social Worker fluent in English and Mandarin Chinese, Cantonese Chinese, Korean, or Vietnamese to support the Brooklyn Case Management Program. The role involves conducting client assessments, creating individualized service plans, providing counseling, and connecting clients with benefits and community resources.

Responsibilities include service coordination, outreach, and documentation.

Competenze

• MSW required and LMSW preferred but not required.
• Fluency in English and at least one of Mandarin Chinese, Cantonese Chinese, Korean, or Vietnamese.

Mansioni

• Conduct comprehensive assessments in clients' homes or office settings, develop written case plans, and provide counseling and guidance.
• Screen clients for eligibility for benefits and coordinate referrals to services such as transportation, home care, Meals on Wheels, and public assistance.
• Coordinate outreach, prepare program reports, and supervise junior staff or interns as assigned.
