JASA in Brooklyn is seeking a Social Worker to support older adults in the Brooklyn Case Management Program. Responsibilities include assessments, service planning, counseling, and connecting clients with benefits.

Fluency in English plus Mandarin Chinese, Cantonese Chinese, Korean, or Vietnamese is required. Schedule is Monday–Friday, 9:00 AM–5:00 PM, 35 hours per week. Language skills help build trust and access services for clients.

Qualificações

• MSW required for the position.
• Fluency in English and at least one target language is required.
• LMSW licensure preferred but not required.
• Strong written and verbal communication skills.

Responsabilidades

• Conduct client assessments in clients' homes and/or office settings.
• Develop and maintain written case plans tailored to each client's needs and goals.
• Provide counseling, guidance, and education to clients and families in multiple languages.
• Screen clients for eligibility for benefits and other assistance programs.
• Coordinate referrals to transportation, home care, Meals on Wheels, and other services.
• Maintain accurate client records and documentation.
