Urban Health Plan in the Bronx seeks a Specialty Case Manager (Bilingual English/Spanish) to join the Social Services team, supporting patients with social assistance, referrals, and advocacy.

This entry-level role requires a Bachelor's in Social Work or related field, fluency in Spanish and English, and comfort with Microsoft Office. Hours are Monday–Friday, onsite, with comprehensive benefits.

Qualifications

• Bachelor's Degree in Social Work, Psychology, Human Services, or related field.
• Bilingual Spanish/English required.

Responsibilities

• Provides case management assistance to patients as assigned.

• Advocates for patients to obtain resources or services.
• Collaborates with coordinators and team members to support patients.
