Job summary

Jewish Association Serving the Aging (JASA) in Brooklyn is seeking a compassionate Social Worker for the Brooklyn Case Management Program. You will conduct home and office assessments, develop individualized service plans, provide counseling, and connect clients with benefits and community resources to help them maintain independence and safety.

Responsibilities include client assessment, service coordination, outreach, and supervision of junior staff.

Qualifications

• MSW or equivalent master's degree in social work or related field.
• MSW required; LMSW preferred but not required.
• New York State certification preferred.
• Excellent written and verbal communication.
• Strong organizational skills and ability to manage multiple clients.
• Ability to travel throughout the community using public transportation.
• Fluency in English; knowledge of other languages helpful.

Responsibilities

• Conduct comprehensive client assessments at home or office.
• Develop and maintain written case plans tailored to each client's needs, circumstances, and goals.
• Provide counseling, guidance, and education to clients and families.
• Screen clients for eligibility for benefits, entitlements, and other available assistance programs.
• Reassess client needs and monitor services to ensure that appropriate support is provided.
• Maintain accurate, timely, and complete client records and case documentation.
• Coordinate referrals and services, including transportation, home care, Meals on Wheels, public assistance, Medicare, Medicaid, emergency financial assistance, legal aid, protective services, vocational placement, medical and psychiatric services, and housing assistance.
• Assess clients' needs related to decision-making capacity, financial exploitation, and other potential risks.
• Assist clients with financial management needs and, when appropriate, explore guardianship or other arrangements to protect their interests.
• Help eligible clients access and coordinate JASA and NYC Aging relief funds.
• Connect clients with community resources and advocate on their behalf to help address barriers to services.
• Communicate significant changes in client circumstances and urgent concerns to the appropriate supervisor.
• Participate in meetings, trainings, and other professional development activities.
• Represent JASA at community meetings and relevant professional gatherings as needed.
• Conduct outreach to inform older adults and community members about available services.
• Prepare reports, maintain program statistics, and contribute to required performance metrics.
• Provide supervision or guidance to bachelor's-level staff and interns, as assigned.
