• Master's degree in Social Work required.
• Associate Clinical Social Worker Licensure required.
• Fluency in Chinese (Mandarin or Cantonese) preferred.

Responsabilités

• Plan, organize and implement comprehensive social work services for PACE participants.
• Conduct psychosocial assessments, develop individualized care plans, and provide ongoing case management.
• Coordinate care with physicians, nurses, therapists, caregivers and community providers to ensure integrated care.
• Maintain accurate clinical documentation and participate in quality improvement activities.
• Travel to patients and care sites within the San Gabriel Valley as needed.
