Job summary

North Bay Regional Center (NBRC) is seeking a Service Coordinator to manage delivery of services for clients with developmental disabilities. You will work under a Case Management Supervisor to implement Individual Program Plans and coordinate community resources.

The role includes occasional supervisory duties and ensuring service delivery aligns with authorizations. Ideal candidates have a Master’s or a qualifying Bachelor's with three years in the field; bilingual English/Spanish is

Qualifications

• Master’s degree in a field related to developmental disabilities.
• Fluency in a language in addition to English is desirable and may be required.
• Ability to provide personal automobile transportation during working hours and emergencies.

Responsibilities

• Under the general direction of a Case Management Supervisor, responsible for the full range of case management duties including intensive case management.
• Implements the clients’ service plans and assists clients in using resources before using regional center funds.
• Conducts annual reassessments of clients’ developmental status with the interdisciplinary team.
