Job summary

Join La Frontera EMPACT - Suicide Prevention Center as a Case Manager in Apache Junction. This full-time position focuses on supporting mental health adults through case management and crisis intervention.

The ideal candidate has a Bachelor’s in a related field, is bi-lingual (Spanish), and possesses a valid AZ driver's license. Generous PTO, medical and retirement plans, and both long-term and short-term disability are part of the benefits offered.

Qualifications

• Valid AZ driver’s license with no major infractions in the past three years.
• Fingerprint clearance card or eligibility to obtain one.
• Flexible work hours and ability to meet clients in various locations.

Responsibilities

• Ensure quality service delivery through direct and community services.
• Facilitate Child and Family Team Meetings and maintain documentation.
• Deliver at least 40 hours of work per week with direct client contact.
• Support services according to the 12 Arizona Principles.
• Maintain accurate client records as per agency policies.
• Seek regular supervision for best practices.
