Job summary

LifePath Systems is seeking a Service Coordinator to manage caseloads, ensure documentation is current, and collaborate with families and stakeholders. The role supports individuals with disabilities in navigating available services, developing plans, and coordinating resources to promote independence and community participation.

The ideal candidate uses initiative, demonstrates cultural and linguistic sensitivity, and works with a team to staff cases weekly.

Qualifications

• Bachelor’s or advanced degree from an accredited college or university preferred.
• Associate degree in a social, behavioral, human service, or health-related field plus one year of paid or unpaid experience with individuals with intellectual or developmental disabilities.
• High school diploma with two years of paid or unpaid experience with individuals with IDD.
• Must have reliable transportation and a valid driver’s license.
• Work experience in a LIDDA or IDD provider services preferred.

Responsibilities

• Coordinate services for individuals on caseload and ensure paperwork is up to date.
• Communicate effectively with families and stakeholders.
• Attend weekly staff meetings to staff individuals served and receive leadership feedback.
• Link individuals to resources and assist in navigating services.
• Maintain trauma-informed, person-centered interactions.
