Job summary

Active Day is seeking a Social Services Coordinator to plan, organize, develop and deliver social services for members and families. This role covers assessments, case management, and collaboration with the clinical team to ensure quality care within regulatory guidelines.

The position requires English and Spanish proficiency, a social work degree (MSW/BS), and at least 1 year of relevant healthcare experience, preferably with seniors or disabilities.

Qualifications

• English and Spanish proficiency required.
• Degree in Social Work (MSW or BS) required.
• Minimum 1 year social work experience in a health care facility, preferably with seniors or disabilities.
• Knowledge of state and federal OSHA regulations.

Responsibilities

• Provide case management coordinating with internal and external services for members and families.
• Coordinate with Nurse Manager and clinical team on member care plans.
• Participate in Safety and Quality Assurance Committees and schedule meetings for Member Council and caregiver groups; document notes.
• Evaluate referrals to determine admission suitability and assist with external referrals as needed.
• Assist members with financial resources and access to appropriate resources.
• Ensure compliance with company policies and regulatory authorities and prepare paperwork.
• Maintain formal written communication with members and families; provide billing, transportation, and referral information.
• Engage in community outreach and center marketing activities.
• Assist members with daily needs (hygiene, meals, ambulation, activities).
• Note: duties may expand as needed by management.
