Job summary

Klamath County seeks a Spanish-speaking Services Coordinator to provide targeted case management for individuals with I/DD within the Developmental Disabilities Services team. The role requires fluency in Spanish and knowledge of Oregon public service systems.

A bachelor’s degree in behavioral or social sciences or related field, or equivalent experience in human services, is preferred. You will develop ISPs/Annual Plans, coordinate with agencies, maintain confidentiality, and support clients

Qualifications

• Bachelor's degree in behavioral or social sciences or related field.
• One year of human services related experience with a bachelor's degree in any field.
• Associate’s degree in behavioral/social sciences with two years of related experience.

Responsibilities

• Develop and revise an ISP or Annual Plan based on assessments and goals.
• Support individuals and families in developing and implementing a support plan.
• Coordinate with agencies to arrange services and resources.
• Maintain knowledge of laws, rules, contracts, and regulations relevant to DD services.
• Document interactions and maintain confidentiality and professional boundaries.
• Attend meetings, supervision, and training, and participate in transition planning for IEPs.
