Job summary

Humana is seeking a Bilingual RN Field Care Manager to assess members’ needs and help them access resources to achieve optimal health. This remote role requires fluency in English and Arabic or Chaldean and a strong background in case management and HCBS.

Responsibilities include managing a caseload, coordinating care plans, and providing clinical guidance for patients with complex needs. Travel within Michigan is extensive, with a full-time schedule and a sign-on bonus eligible plan.

Qualifications

• Active Michigan RN or APRN license required (NP or CNS acceptable).
• 2+ years in healthcare and/or case management.
• Fluent in English and Arabic or Chaldean, spoken and written.

Responsibilities

• Assess members’ needs and help them access appropriate resources.
• Manage a caseload and complete member assessments in various settings.
• Provide clinical support and guidance for members with complex medical needs.
• Develop and coordinate care plans to manage members' health effectively.
• Address barriers to healthcare and advocate for optimal member outcomes.
• Conduct comprehensive InterRAI Integrated Health Care assessments.
• Serve as the primary contact for the Interdisciplinary Care Team.
• Review and complete medical complexity attestations and clinical oversight activities.
• Develop and update the Individual Care Plan with care team members.
