Job summary

Humana Inc. seeks a LTSS Bilingual Field Care Manager, Social Worker in Michigan to assess and plan care for members in home and community settings, with a $5,000 sign-on bonus.

This role requires Arabic and/or Chaldean Neo-Aramaic fluency and a current Michigan social work license, with 2+ years post-degree experience. Qualifications

• Must reside in Michigan and be able to travel throughout the assigned service area, including Wayne and/or Macomb Counties.
• Fluent in English and Arabic or Chaldean Neo-Aramaic, with the ability to speak, read, and write each language independently and without assistance.
• Active Michigan Social Work license in good standing required (LBSW, LLMSW, or LMSW). Candidates with a limited license must demonstrate active progress toward full licensure.
• 2+ years of post-degree experience in social work, behavioral health, health care, care management, or community-based services.
• 1+ year of experience supporting Home and Community-Based Services (HCBS) and/or waiver programs.
• Experience conducting Long-Term Services and Supports (LTSS) assessments.
• Knowledge of LTSS, HCBS, Medicaid requirements, person-centered planning principles, member rights, and supporting members in the least restrictive setting.
• Strong documentation skills and proficiency with EMRs and care management platforms.
• Strong verbal and written communication skills with the ability to build effective relationships with members, families, caregivers, providers, and community partners.

Responsibilities

• Assess and coordinate LTSS needs and care planning for members in home/community settings.
• Conduct InterRAI Integrated Health Care assessments to evaluate functional, cognitive, behavioral, environmental, and psychosocial needs.
• Develop, implement, monitor, and modify individualized person-centered care plans in collaboration with the member and care team.
• Serve as primary contact for the Interdisciplinary Care Team and coordinate with care team members and external resources.
• Identify and address urgent health, safety, or service-access concerns per policy and regulatory requirements.
• Complete timely, accurate documentation of assessments, care plans, contacts, interventions, referrals, and outcomes in designated systems.
