Job summary

Humana is seeking a LTSS Bilingual Field Care Manager, Social Worker to assess and coordinate services for Michigan members in home or community settings. You will guide families, coordinate resources, and ensure culturally responsive, person-centered care planning.

Responsibilities include comprehensive assessments, care plan development, and collaboration with interdisciplinary teams. Requires bilingual English and Arabic or Chaldean Neo-Aramaic, plus active Michigan social work license.

Qualifications

• Assessment and care planning in LTSS/HCBS/Medicaid environments.
• Proficiency with InterRAI assessments to guide person-centered care planning.
• Experience coordinating with interdisciplinary care teams and community resources.

Responsibilities

• Manage caseload and perform comprehensive assessments in home or community settings.
• Conduct InterRAI assessments and collaborate on care determinations.
• Develop and monitor individualized, person-centered care plans.
• Coordinate with care team, providers, and caregivers to meet needs.
• Identify social determinants of health and access barriers.
• Document assessments, plans, contacts, and outcomes in EMR.
• Provide ongoing care coordination and follow-up.
