Job summary

Humana is seeking a LTSS Bilingual Field Care Manager, Social Worker in Michigan to assess members' needs, plan care, and guide families to resources for health and independence.

You will perform InterRAI assessments, develop person-centered plans, and coordinate with the interdisciplinary team to coordinate Medicaid, HCBS waivers, and community services, while communicating in English and Arabic/Chaldean Neo-Aramaic.

Qualifications

• Must reside in Michigan and be able to travel throughout the assigned service area.
• Fluent in English and Arabic or Chaldean Neo-Aramaic, speak/read/write each language independently.
• Active Michigan Social Work license in good standing (LBSW/LLMSW/LMSW).
• 2+ years post-degree experience in social work or related field.
• 1+ year HCBS/waiver program experience.

Responsibilities

• Manage assigned caseload and complete comprehensive assessments.
• Conduct InterRAI Integrated Health Care assessments for person-centered planning.
• Identify clinical findings requiring RN review or intervention.
• Coordinate with nursing staff for Level of Care determinations.
• Develop and monitor individualized care plans with Interdisciplinary Care Team.
• Coordinate with members, caregivers, and providers to address needs.
• Assess social determinants of health and barriers to services.
• Facilitate access to Medicaid, Medicare, HCBS, and community resources.
• Ensure services in least restrictive, integrated settings.
• Provide ongoing care coordination and advocacy.
• Apply trauma-informed and culturally responsive engagement techniques.
• Collaborate with care team and community partners.
