Job summary

Evry Health is seeking an RN Integrated Care Coordinator to deliver comprehensive, member-centered care coordination for our fully insured commercial population in a remote, technology-forward environment. You will engage proactively and reactively, develop individualized care plans, and educate members and providers on benefits and self-management strategies.

Requirements include Texas RN license (or Compact), 3–5 years direct patient care plus 1–2 years in health plans, and bilingual

Qualifications

• RN license (Texas or Compact) required.
• 3–5 years of nursing experience in direct patient care, plus 1–2 years in a health plan or managed care organization.
• Bilingual English/Spanish fluency required.
• Diploma in nursing required; ADN or higher preferred.
• Telecommuting: remote role with dedicated work area and privacy.
• BSN preferred; CCM/ACM certification or UM background advantageous.

Responsibilities

• Proactive and reactive member engagement based on risk stratification and care gaps.
• Develops individualized care plans and coordinates resources across the care continuum.
• Educates members and providers on benefits, digital health solutions, and self-management.
• Supports utilization review as a bridge between UM team, providers, and members.
• Documents care coordination activities and manages escalations; promotes digital health program enrollment.
