Alignment Health is seeking an RN Case Manager – Transitions of Care for a 100% remote role. The position requires an active California RN license and bilingual ability in Spanish or Vietnamese, with a focus on coordinating care across providers and educating members and families.

Qualifications

• Active, unrestricted California RN license.
• Bilingual in Spanish or Vietnamese.
• 2+ years of case management or clinical experience.
• Willing to obtain licensure in other designated states within 6 months (licensure fees reimbursed).
• Strong communication and care coordination skills.
• Preferred: Case Management Certification.

Responsibilities

• Coordinate services across providers and community resources.
• Educate members and families on diagnoses, medications, and self-care.
