Job summary

PharmD Live seeks a bilingual Care Coordinator (RN/LPN) to support RPM and CCM programs remotely. You will engage patients, review data, and coordinate care to reduce hospitalizations. This role emphasizes patient education in Spanish and English, and collaboration with clinicians to optimize outcomes.

The position requires an active Tennessee RN or LPN license, 2+ years in telehealth or chronic disease management, and strong proficiency with EHRs in a HIPAA-compliant remote setting.

Qualifications

• Active Tennessee RN or LPN license required.
• Bilingual in Spanish and English (oral & written) required.
• 2+ years in care coordination, telehealth, or CCM.
• Experience with RPM/CCM programs and remote care.

Responsibilities

• Serve as primary contact for RPM/CCM patients.
• Review daily health data, identify red flags, escalate as needed.
• Provide culturally competent health education in Spanish and English.
• Collaborate with physicians, pharmacists, and interdisciplinary teams.
• Coordinate follow-ups, referrals, and care transitions.
• Document interactions in the EHR per CMS regulations.
• Support program enrollment and patient onboarding for RPM devices.
