Job summary

TEKsystems in Texas (remote) is seeking an experienced Registered Nurse to transition from bedside care into care coordination, member advocacy, and improved health outcomes. This fully remote role supports high-risk populations while balancing work-life integration.

The ideal candidate has a Texas RN license, 3+ years in community/public health or case management, and experience coordinating care with primary care providers; outpatient case management in payer/managed care is preferred.

Qualifications

• Must have Texas RN license active/unrestricted.
• Must reside in Texas.
• 3+ years in community/public health, chronic disease management, or case management.
• 1+ year in care coordination and disease management/patient education.
• 3+ years coordinating care with primary care providers.
• Outpatient case management experience in a health insurance, payer, or managed care environment.

Responsibilities

• Conduct telephonic assessments for high-risk members.
• Develop individualized care plans and measurable health goals.
• Coordinate services between providers, behavioral health teams, community resources, and members.
• Educate members on chronic disease management, preventive care, medications, nutrition, and healthy lifestyle choices.
• Support transitions of care and remove barriers to treatment.
• Maintain accurate documentation and ensure regulatory compliance.
