Job summary

Careforth is seeking an RN Care Coach to join our care management team in the United States. Work Monday through Friday, 11:00 AM to 8:00 PM EST, guiding families through initial assessments, eligibility determinations, and care plans to activate services.

This remote-first role emphasizes education for caregivers, coordination of assessments, and collaboration with case management partners, with opportunities to provide ongoing coaching and ensure regulatory compliance.

Qualifications

• Must hold an active RN license in the state(s) of employment.
• Minimum two years of direct care experience for elders or adults with disabilities.
• Experience in case management, care planning and assessment for eldercare and other complex medical needs.
• Proficiency with common software and web-based applications (e.g. MS Office).
• Fluency in English and Spanish required.
• ER experience and triage preferred.

Responsibilities

• Serve as the initial contact of the coaching team and guide the caregiver and consumer through the referral process.
• Educate prospective families.
• Coordinate and complete all applicable assessments.
• Coordinate service activation and ensure forms and procedures are followed.
• Develop relationships with case management entities to drive collaboration.
• Provide ongoing coaching to the caregiver with the team as needed.
• Develop, review and approve each member’s plan of care on an ongoing basis.
• Provide clinical consultation and training to coaching team.
• Ensure survey readiness and regulatory/accreditation compliance.
• Perform other duties as assigned.
