Careforth, Inc. in the United States is seeking an RN Care Manager to provide personalized care and support to caregivers and their loved ones. The role includes home visits, coaching via our app, and developing individualized care plans to improve consumer care.

You'll conduct home visits within your territory, coach caregivers, and document records to ensure regulatory compliance. This remote-first, travel-friendly position offers a flexible schedule and no night shifts or weekends.

Qualifications

• Active RN license in the state of employment.
• Minimum two years of direct elder/adult disability care experience.
• Ability to travel with valid driver's license and insurance.
• Experience in care planning, assessment and case management.
• Proficiency with MS Office and care-management tools.

Responsibilities

• Conduct home visits to assess safety and health status and review medications.
• Provide ongoing coaching to caregivers via app and refer resources.
• Develop trusting relationships to reduce caregiver burnout.
• Document and review records for accuracy and regulatory compliance.
• Other duties as assigned.
