Job summary

Careforth is seeking an RN Care Manager to support family caregivers and their loved ones at home across the United States. You will assess needs, build individualized care plans, and conduct home visits while coaching caregivers via our app.

This role offers flexibility and a remote-first culture with a strong focus on consumer care. Required are an active RN license and at least two years of direct elder or disability care, plus travel within the assigned territory.

Qualifications

• Active RN license in the state of employment.
• Minimum two years direct care of elders or adults with disabilities.
• Ability to travel within assigned geographic area; valid driver’s license and automobile insurance.
• Experience in case management, care planning and assessment.
• Proficiency in MS Office and case management systems.
• Additional state-specific qualifications may be required.

Responsibilities

• Conduct home visits to assess home safety and health status changes.
• Provide ongoing coaching to caregivers via app, including resource referral.
• Develop trusting relationships with caregivers to reduce burnout.
• Document and review records for accuracy and timeliness, ensuring compliance with standards.
• Other projects and duties as assigned.
• Perform other duties as assigned.
