Job summary

A healthcare technology company is seeking a Bilingual Registered Nurse Care Coach to work remotely part-time. The RN will manage a group of Medicare patients, providing education and support to improve patient outcomes, while ensuring effective documentation and care coordination. Candidates must be fluent in English and Spanish with at least 3 years of RN experience. Competitive pay at $15 per patient encounter, with bonus eligibility after 3 months.

Responsibilities

• Utilize specialized care management software to call Medicare patients monthly.
• Build and maintain rapport with patients to coach them to improved health.
• Implement and update the Plan of Care by tracking medications and appointments.

Strong communication and telephonic skills

Strong critical thinking and problem-solving skills

Education

Current, unrestricted RN Compact License or New Mexico RN license

3+ years' experience as a Registered Nurse
