Job summary

HumanGood is seeking a compassionate Resident Services Coordinator in Delaware to organize, implement and evaluate social services, assessing residents' psychosocial needs and linking them to supports that help them live independently in the community.

This full-time role requires 1+ year of case management, 2+ years of social services experience (preferably in senior care), and a Bachelor's in social work or related field; bilingual English/Mandarin is a plus.

Qualifications

• 1+ years of experience in case management.
• 2+ years of social services experience, preferably in a senior care setting
• A Bachelor’s Degree required in social work or related program such as gerontology, psychology, or recreation therapy
• Bilingual - English/Mandarin
• Social Services certification preferred

Responsibilities

• Assess resident needs based on admission social services history, resident’s health, psychological and social needs
• Promote a positive living experience for our residents
• Ensure confidentiality by adhering to HIPAA & all other privacy standards
• Promote a positive work environment by collaborating with community team members
