Job summary

Sequoia Living in San Francisco seeks a bi-lingual Resident Services Coordinator to provide outreach, assessments, and care planning for seniors and disabled adults. The role involves liaison work, case management, information/referral, and collaboration with housing staff.

Training related to Narcan distribution may be required. The position offers a salary of $34.33 per hour, 40 hours weekly, with benefits such as medical/dental coverage, life insurance, and tuition assistance opportunities.

Qualifications

• Bachelor's degree in Social Work, Sociology, Gerontology, Psychology, or related field required.
• Master's degree in related field preferred.
• Minimum of three (3) years of recent experience in a social services capacity.
• Some sites may require bilingual capacity.
• Ability to drive with CA license, insurance, and clean driving record.

Responsibilities

• Establishes and maintains relationships with residents, families and neighborhood groups.
• Identifies residents in need of services and documents health status.
• Serves as liaison with outside social and health agencies.
• Provides casework and care plan development and coordination of services.
• Maintains records and ensures documentation and statistics.
