Job summary

Phipps Houses and Affiliates is seeking a dedicated case manager to support residents with intake, education on available services, and referrals to community providers. The role includes translation support and coordinating with service agencies to aid housing stability for seniors and disabled residents.

Responsibilities include monitoring service delivery, organizing community events, and assisting with recertification and eligibility processes.

Qualifications

• Minimum of Diploma or GED is required.
• College degree in social work is a plus.
• Two to three years' experience in social service delivery with seniors or people with disabilities.
• Demonstrated working knowledge of supportive services and other resources in the area served by the project.
• Demonstrated ability to advocate, organize, problem-solve and provide results for families.
• Experience with HUD multifamily housing and annual requirements
• Bi-lingual (Spanish) preferred

Responsibilities

• Provides general case management including intake, education on services, and referral to community service providers.
• Provides translation services through Phipps contractors for non-English speakers.
• Collects Service Agency information for referrals and connects residents to assistance programs.
• Sponsors informational events on health care, job search, life skills, etc.
• Facilitates referrals of Self-Help Groups within the community.
• Monitors service provision by community agencies and maintains up-to-date case management.
• Sets up volunteer support programs with service organizations.
• Helps residents build informal support networks with others.
• Assists with recertification documents and eligibility determinations to maintain housing stability.
• Provides services for formerly homeless families.
• Conducts home visits for residents with mobility limitations or housing benefit considerations.
