Job summary

Fair Haven Community Health Care in New Haven, CT is seeking a compassionate Referrals Navigator to support patients and clinical teams through the referral process from initiation to follow-up.

You will handle prior authorizations, schedule care at referral sites, document in the EHR, educate patients on requirements, and collaborate with supervisors to improve performance while ensuring privacy and patient-centered care.

Qualifications

• One year of experience in a health care setting.
• Referrals experience preferred.
• Proficient data entry using Excel or Access.
• Excellent communication and teamwork.
• Fluency in English and Spanish.

Responsibilities

• Manage all aspects of external referrals with clinical teams.
• Obtain prior authorizations when needed.
• Schedule appointments and coordinate with referral sites.
• Document referrals in the patient’s EHR and inform clinicians of updates.
• Educate patients about referral requirements and sites.
• Educate clinicians on changes to the referrals process.
• Track and follow up on referrals and performance metrics.
• Maintain a searchable referrals database as directed.
• Identify access barriers and resolve referral issues.
• Maintain knowledge of insurance practices and payer policies.
• Provide coverage for front desk and other departments as needed.
