Job summary

Yale New Haven Health seeks a Lead Patient Navigator to supervise staff, train new navigators, and coordinate enrollment for PA‑NH patients in the New Haven area. The role includes managing data systems, supporting program expansion, and serving as a key liaison between the community, PA‑NH, and YNHH.

The ideal candidate has 3+ years in health care, bilingual Spanish, and experience with patient navigation and data management.

Qualifications

• Associates degree required; bachelor’s in psychology, sociology, health and/or human services preferred.
• At least 3 years in health care or human services; strong communication and outreach experience.
• Bilingual Spanish is required.

Responsibilities

• Supervise staff and act as go-to contact for navigation team questions.
• Train new PA-NH navigators and staff on Clara Data Management System.
• Coordinate patient appointments, enrollment, reevaluation, and referrals.
• Maintain accurate patient records and document interactions in electronic systems.
• Participate in outreach, training, and community partnership activities.
