Job summary

Paylocity is seeking a Referral Coordinator to process referrals and coordinate care for designated patients between primary care providers, specialists, and other involved entities. You will work with the care team during and after patient visits, facilitate appointments, and ensure timely communication to support adherence to the care plan.

The role requires at least a high school diploma and 1 year of customer service/referral coordination experience.

Qualifications

• High school diploma or equivalent.
• 1 year of experience in customer service/referral coordination.
• Experience with health care programs preferred.
• Experience serving underserved and diverse populations preferred.
• Basic medical terminology knowledge preferred.
• Electronic Health Record (EHR) experience preferred.

Responsibilities

• Pre-Visit Preparation: Facilitate patient appointments, obtain consult notes, follow up with patient/family and coordinate care.
• During Patient Visit: Review care plan goals with patient/family and provide resources to connect to specialists or services.
• Between Patient Visits: Notify patients of referral status updates and ensure proper closure of the referral loop in the EHR.
• Maintain confidentiality and HIPAA compliance in all communications.
