Mary’s Center is seeking a Referral Care Coordinator to facilitate timely referrals for clients and connect families to internal and external services. The role emphasizes intake, follow-up, and resource navigation while upholding privacy and strong partner relationships.

You will educate participants about services, coordinate referrals, and maintain accurate records in the EHR system, ensuring HIPAA compliance and trauma-informed, language-accessible support.

Qualifikationen

• Requires a minimum of High School Diploma or GED and one year of experience in patient registration or as a medical assistant.
• Must be proficient in PC usage and bilingual in English and Spanish.
• Knowledge of HIPAA privacy regulations is essential.

Aufgaben

• Coordinate timely and accurate referrals for Mary’s Center clients.
• Answer referral telephone line and voicemail promptly.
• Educate participants to ensure referral appointment compliance.
• Maintain up-to-date participant records in the electronic health/records system.
• Serve as primary contact for participants seeking referral services.
• Build relationships with community organizations to streamline referrals.
• Identify barriers to access and connect participants with support.
• Participate in case conferences and team meetings to coordinate care plans.
• Develop and maintain referral guidelines and procedures.
• Ensure data integrity and HIPAA compliance.
• Support site leadership with program needs.
• Perform other duties as assigned.
