Job summary

Colorado Coalition for the Homeless is seeking a Bilingual Case Manager to provide stabilization and time-limited case management for English and Spanish-speaking clients exiting homelessness. You will work with a multidisciplinary team to ensure rapid exits to housing and up-to-date follow-up care for the first year.

You will engage in trauma‑informed support, crisis intervention, basic needs provision, medical and behavioral health facilitation, transportation, and client advocacy.

Qualifications

  • High School Diploma or GED required.
  • Bachelor’s degree in Psychology, Social Work, Human Services, Criminal Justice preferred.
  • Basic Life Support Certification required within 30 days of employment.
  • Colorado driver’s license required within 30 days of hire.
  • 1–2 years of experience working with a diverse population experiencing homelessness, substance use disorders or mental health challenges preferred.

Responsibilities

  • Manages a caseload of clients with high-service utilization and disabilities.
  • Maintain positive and co-working relationships with community partners and health providers.
  • Works under direct supervision with set department parameters and collaborates within a team.
  • Support clients with wrap-around care management to ensure consistent communication and engagement.
  • Acts as a liaison between internal program management, client referral, and community partners.
  • Respond promptly to crises involving residents, providing de-escalation, support, and referrals.
  • Develop and implement crisis and safety plans tailored to client needs.
  • Mediates conflicts to maintain positive relationships.
  • Tracks client data, engagement, and outcomes to improve individual results.
  • Transports clients using a CCH vehicle or approved personal vehicle; accompany residents to appointments.
  • Outreach to assess basic needs, safety, and mental health resource needs.
  • Assist clients in applying for and securing benefits (e.g., Social Security, Medicaid, food assistance).
  • Coordinate referrals to external agencies and services as needed.
  • Participate in community collaboration meetings to address barriers and support housing exits.
