Job summary

Qualifications

• Associate's degree or equivalent experience in a related field.
• Experience with families of individuals with developmental disabilities preferred.
• Familiarity with Inland Regional Center processes preferred.
• Proficient in Microsoft Office and EHR systems.
• Ability to conduct home and virtual visits with HIPAA compliance.

Responsibilities

• Conduct scheduled and surprise in-home visits to assess care quality and address concerns.
• Complete documentation and update databases following visits.
• Perform biannual phone check-ins and 30-day post-staffing follow-ups with families.
• Monitor client utilization and report trends and concerns.
• Engage families to prevent service termination and support retention.
• Coach families on requesting additional hours or services.
• Track unused service hours and advise on maximizing usage.
• Maintain professional, compassionate communication while ensuring HIPAA compliance.
