Job summary

Yale New Haven Health is seeking a Lead Community Health Worker to guide navigation initiatives, collaborate with internal and external partners, and improve patient pathways across the health system. The role reports to the Manager, Patient Navigation and supports CHW staff development.

Ideal candidates will have 3+ years in healthcare or human services, an Associate's Degree, and the ability to obtain CHW certification within a year.

Qualifications

• Minimum of an Associate's Degree with community health exposure.
• At least 3 years' experience in health care or human services.
• Experience as case manager, CHW, or patient navigator preferred.
• State CHW Certification preferred or ability to obtain within 1 year; valid driver's license required.

Responsibilities

• Lead CHW navigation programs and coordinate with internal and external stakeholders.
• Provide problem solving navigation for high‑risk patients and monitor referrals.
• Develop training materials and schedule for CHW staff and ensure milestones are met.
• Maintain accurate patient records and supervise CHW team activities.
• Engage community partners and participate in quarterly meetings to evaluate referrals.
