Colonial Group is seeking a Pediatric Community Health Worker (CHW) to provide care coordination, education, and navigation for infant and toddler members and their families in California. This in-person, field-based role requires travel to members’ homes, medical offices, and county facilities.

The CHW will connect members with primary care, address social determinants of health, and collaborate with social services to improve health outcomes and equity.

Qualifikationen

• The CHW must hold a CHW certification or complete the certification within 18 months of hire.
• An associate or bachelor's degree in public health, psychology, social work, sociology or related field is required.
• Direct work with Medi-Cal populations or community-based settings is preferred.
• Certification programs should include field experience and SDOH knowledge.

Aufgaben

• Maintain a caseload and coordinate services between members and providers.
• Provide health education, navigation, and referrals to improve physical and mental health.
• Serve as liaison with primary care offices and community resources for care coordination.
• Document activities promptly and track progress in care management systems.
• Travel to member homes and various locations up to 85% of the time.
