Health Plan of San Mateo (HPSM) is seeking a Pediatric Community Health Worker to provide in-person care coordination, health education and navigation for infant to toddler members and their families. The role requires travel to member homes, primary care offices and community locations to improve health equity and access to services.

Qualifikationen

• CHW certification pathway or equivalent experience with requirement to obtain CHW certification within 18 months.
• Degree in justice or social services with direct Medi-Cal population experience preferred.
• Fluency in Spanish required to communicate with a diverse member population.
• Strong knowledge of Medi-Cal benefits and CHW program basics.
• Valid CA driver’s license and ability to travel throughout California up to 85%.

• Maintain a caseload and coordinate services among providers and community agencies.
• Provide health education, outreach and navigation to perinatal and family health needs.
• Administer assessments and support care plan goals with member centric approach.
• Serve as liaison, ensure timely documentation and reporting in care management systems.
