The CHW will collaborate with clinic staff, build trust in diverse communities, provide culturally attuned education, and help families access insurance, appointments, and resources in Pierce County.

Bilingual preferred.

Qualifications

• Education: High School Diploma or GED.
• Experience navigating social service organizations.
• Licensure: Valid driver's license and current auto insurance.
• Bilingual preferred.

Responsibilities

• Collaborate with clinic staff to engage children, youth, and families.
• Identify service needs via interviews and maintain care plans.
• Connect families to healthcare services and assist with enrollment.
• Provide culturally appropriate health education and advocacy.
• Attend staff meetings, collect data, and participate in professional development.
• Support case conferences and interdisciplinary teams.
