Job summary

LifeLong Medical Care is seeking a Perinatal and Pediatric Community Health Worker (CHW) for our West Berkeley Family Practice in Berkeley, CA. The CHW will join a multidisciplinary team to provide perinatal and pediatric services, including case management, outreach, health education, and care coordination for CPSP programs.

The role is full-time (40 hours/week), with English/Spanish bilingual requirements and benefits eligibility.

Qualifications

• Strong communication skills, both oral and written.
• Bilingual English/Spanish required.
• Ability to prioritize competing tasks from clients and staff.
• Ability to work calmly under pressure in a positive manner.
• Sensitivity working with diverse populations and health determinants.
• Experience in perinatal or maternal/child health settings preferred.

Responsibilities

• Manages caseload of perinatal clients and their infants, scheduling appointments and registering newborns.
• Provides case management, lactation support and care coordination for specialty services as needed.
• Coordinates cases with social service agencies and medical providers.
• Refers clients to appropriate internal and community support services.
• Provides CPSP services through individual appointments and referrals.
• Documents CPSP visits and patient interactions in the EHR system.
• Performs outreach calls and tracks Well Child Checks and immunizations.
• Supports and co-facilitates health education classes.
• Attends team and staff meetings and participates in outreach as needed.
• Complies with data collection for quality improvement and annual reports.
