• Document all patient interactions in designated systems with accuracy.

We are seeking a compassionate, organized, and results-driven Patient Outreach Coordinator to support population health and quality improvement initiatives through proactive patient engagement. This role is responsible for conducting outreach to patients, scheduling preventive services, closing care gaps, and supporting HEDIS, STAR, and Risk Adjustment initiatives. The ideal candidate is comfortable interacting with patients, navigating healthcare systems, and collaborating with clinical and operational teams to improve health outcomes.

This position is full-time (40 hours/week) Monday - Friday. Employees are required to work an 8-hour shift schedule during our normal business hours of 8am - 5pm PST. It may be necessary, given the business need, to work occasional overtime. Our office is located at 9 Technology Drive, Irvine, CA.

This will be on the job training and the hours during training will be 8am to 5pm, Monday - Friday.

Primary Responsibilities:

• Conduct outbound calls, text messages, and other patient outreach activities to address preventive care and chronic condition care gaps.
• Assist patients with scheduling appointments, screenings, laboratory tests, and follow-up visits with primary care providers and specialists.
• Educate patients on the importance of preventive health services, chronic disease management, and annual wellness visits.
• Support outreach campaigns related to HEDIS, STAR, Risk Adjustment, and other quality improvement initiatives.
• Document all patient interactions accurately in designated systems and maintain outreach tracking records.
• Coordinate with provider offices, health plans, and internal teams to facilitate completion of healthcare services.
• Monitor patient engagement and follow-up activities to ensure timely closure of quality measure gaps.
• Review patient lists and reports to prioritize outreach efforts and identify barriers to care.
• Escalate clinical concerns, social needs, or urgent issues to appropriate clinical staff according to established protocols.
• Assist with mailings, educational campaigns, and special population health projects as assigned.
• Maintain compliance with HIPAA and all organizational privacy and confidentiality requirements.
• Participate in team meetings, training sessions, and performance improvement activities.
• Works in the office minimum two days a week, up to five days at the discretion of the manager

Required Qualifications:

• High School Diploma or equivalent; Associate degree preferred
• 6+ months of experience using Microsoft Excel
• 6+ months of Customer Service Experience using phones and computers as primary job tools
• Experience with computers and Windows PC applications, which includes the ability to navigate and learn new and complex computer system applications
• Bilingual fluency in English and Spanish or Vietnamese

Preferred Qualifications:

• Experience in healthcare, customer service, population health, care coordination, or patient outreach
• Knowledge of medical terminology and healthcare office workflows
• Familiarity with HEDIS, STAR, preventive care measures, and population health programs
• Proficiency in Microsoft Office Suite and experience working with electronic health records (EHRs) and outreach platforms

Telecommuting Requirements:

• Reside within commutable distance to the office at 9 Technology Drive Irvine, CA
• Ability to keep all company sensitive documents secure (if applicable)
• Required to have a dedicated work area established that is separated from other living areas and provides information privacy.
• Must live in a location that can receive a UnitedHealth Group approved high-speed internet connection or leverage an existing high-speed internet service.

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements).
