Job summary

Suvida Healthcare is seeking a Patient Outreach Specialist to engage underserved seniors in Houston through field-based home visits and coordinated care with a multidisciplinary team.

The role emphasizes bilingual English/Spanish communication, strong organization, and the ability to work evenings and weekends to reach hard-to-engage patients and support successful transitions of care.

Qualifications

• High School Diploma or equivalent required.
• Bachelor’s Degree preferred.
• Community Health Worker (CHW) certification preferred.
• ACLS or BLS certification required or willingness to obtain within 7 days of start.

Responsibilities

• Conduct in-home outreach visits to establish relationships and schedule initial appointments.
• Engage patients recently discharged to support safe transitions and continuity of care.
• Collaborate with patients, families, caregivers, providers, and community partners to address barriers to care.
• Coordinate transitions of care with clinical and case management teams.
• Ensure clear, collaborative communication across multidisciplinary teams for patient-centered care.
• Utilize knowledge of Medicare/Medicaid and community resources to support patient needs.
