Job summary

Suvida Healthcare in Houston is seeking a Patient Outreach Specialist to engage patients who have not established care and to support post-discharge transitions. You will work with a multidisciplinary team, perform field home visits, and help patients schedule initial appointments with Suvida providers.

The role requires availability evenings and weekends, bilingual English/Spanish, and 3–5 years in healthcare.

Qualifications

• 3–5 years of experience in a healthcare or community-based setting.
• Experience with geriatric populations preferred.
• Proficiency with EMRs, mobile technology, and Microsoft Office Suite.
• Strong knowledge of Medicare, Medicaid, and community resource navigation.
• Ability to work independently while fostering collaboration across teams.
• Bi-lingual in English and Spanish.
• Valid state driver’s license in good standing.
• Minimum age 25.
• Clean driving record and criminal background.
• Experience in value-based care operations preferred.

Responsibilities

• Conduct in-home outreach visits to establish relationships and schedule initial appointments.
• Engage patients recently discharged from acute care to support safe transitions.
• Collaborate with patients, families, caregivers, providers, and community partners to address barriers to care.
• Coordinate transitions of care with clinical and case management teams.
• Facilitate clear communication across multidisciplinary teams for coordinated care.
• Document patient interactions accurately in Airtable and EMR.
• Support performance goals aligned with value-based care outcomes.
• Mentor and support less experienced staff as appropriate.
• Stay up-to-date on public assistance programs and community resources.
• Assist with daily route planning and field logistics for Houston centers.
• Schedule patients for first PCP appointments during/after home visits.
