Responsibilities

• Conduct in-home outreach visits to establish relationships and schedule initial appointments.
• Engage patients recently discharged to support safe transitions.
• Collaborate with families, caregivers, providers, and community partners to address barriers to care.
• Coordinate with clinical teams on transitions of care and referrals.
• Document interactions in Airtable and EMR accurately and timely.
• Work evenings and weekends as required to engage underserved populations.
