Job summary

Salud Para La Gente, Inc. is seeking a Patient Care Coordinator to support delivery of healthcare visits, coordinate patient paperwork, and ensure timely test notifications. The role involves multi-site coordination within a community health center network and direct patient communication.

The candidate should be bilingual (Spanish/English), have an MA certificate or AA with MA, and possess EMR and scheduling/billing system knowledge. Travel between sites and on-call duties are required.

Qualifications

• MA certificate or AA with MA certificate preferred
• Experience in a clinical setting with physicians/nurses/techs
• Bilingual in Spanish and English required
• Knowledge of EMR, scheduling and billing systems
• Proficiency in Microsoft Office applications (Word/Excel/PowerPoint)
• Valid CA Driver’s License and ability to travel between sites

Responsibilities

• Bridge communication between health center, care team, and patient.
• Process patient paperwork and forms in a timely manner.
• Notify patients of tests and schedule follow-ups.
• Manage provider patient panels and registries.
• Conduct patient recalls and follow-ups after no-shows or discharges.
• Coordinate patient care and assist with referrals and labs.
