We are looking for an Outreach Coordinator to serve as the primary point of contact for families referred by Regional Centers, guiding them through the initial stages of service enrollment and ensuring a seamless onboarding experience. This role coordinates referrals, maintains strong relationships with families and Regional Center staff, supports both Agency and Employer of Record (EOR) service lines, and partners with Community Partnerships and Operations to ensure timely, high‑quality service delivery.
Business hours are 8:00 a.m. to 6:00 p.m. Pacific Time. Depending on the position and business needs, employees may be required to work any scheduled shift within these operating hours. This is a hybrid position, coming into the Concord office up to 3 × per week.

Should withdrawal of a conditional offer of employment be determined, 24 Hour Home Care will provide a copy of the Second Individualized Assessment.

The expected California Pay Range for this position: $21.84 USD - $23.89 USD
