Required: RN licensure in the state, 3+ years of clinical experience; CCM preferred.

Qualifications

• RN licensure in the state of operation.
• CCM certification or credentialing is preferred.
• Fluency in Spanish and English is required.

Responsibilities

• Provide medical case management coordinating with patient, physicians, and employers in both Spanish and English.
• Apply Case Management steps: assessment, planning, implementation, evaluation, and outcomes.
• Evaluate treatment plans for medical necessity and cost-effectiveness.
• Coordinate durable medical equipment and home health services.
• Assess rehabilitation facilities and arrange transportation and home adaptations as needed.
• Communicate medical information clearly to patients and families.
